First Year Student Information
Each first year student-athlete at North Central College is required to submit proof of a recent physical examination and proof of insurance prior to their initial participation in intercollegiate athletics at North Central College.
For students entering North Central College during the 2010-2011 academic year,
you must complete your physical after June 1, 2010.
We highly recommend that the physical take place between June 1, 2010 and August 1, 2010. If you have already received your physical examination for entry into NCC for fall 2010, it may be necessary to have a second physical examination to meet NCAA rules; please contact Bob Simon, Head Athletic Trainer, with any questions.
Due to NCAA rules, all student-athletes must have insurance coverage for athletic related injuries sustained while participating in intercollegiate athletics. Prior to any athletics participation, all student-athletes must complete the Medical History form and provide a copy of their current insurance card to the Athletic Training Staff. Included in the Medical History form is emergency contact information and an opportunity for you to disclose the name and contact information for your primary care physician (if applicable).
We encourage parents/guardians to complete the Medical History form with their son/daughter and explain provisions of the student's personal insurance plan.
Please forward a copy of your Medical History and Insurance Information form, along with a copy of the front and back of your insurance card, to Bob Simon, Head Athletic Trainer, no later than August 1, 2010. To expedite processing, please send all documents together directly to the Athletic Training Department at North Central College, 30 N. Brainard St., Naperville, IL, 60540. We highly recommend that you make an additional copy of these documents and bring them to your first team meeting.
